New Patient Request-- Jan 2024

Having had a practice in the growing Bay Area for over 22 years, our schedules are currently full. We have limited the volume so that we can adequately treat our current patients. We have placed all new requests on hold tentatively until January 2024. Please complete this form starting January 2024 for consideration.

Please note that we do not provide chronic pain management, or specialized psychiatric services, and do not prescribe long term opioids and certain psychiatric medications.  


While completion of form does not establish care with our practice, we will notify you if we are not able to see you within a reasonable time frame.


As a general rule, we do not do "physicals" on the first visit.
