Medical Examination for Adjustment of Status
Surgeon Physical Examination 

 We are able to provide completion of the physical examination requirement for the US government.
The first day involves a review of requirements, the physical examination with tests and vaccines.  The government requires us to order the blood and urine tests.   You can go to any lab, but we have partnered with LabCorp for discount pricing.

 Once all the information is collected-- (you can email results to us), a PDF of the final form will be sent to you for review.  Corrections can be made if required, and once OK, you can phone the office to come in and sign and collect. 

COVID Vaccination approved by the CDC are required to be completed for the I-693

Please bring a government issued picture identification at the initial appointment. 

Adult Required components

Physical Examination

• Tuberculosis blood test  (all applicants above 2 years of age
• Syphilis test (all applicants 18 to 44 years old)
• Gonnorhea urine test (all applicants 18 to 24 years old )
• Tetanus Vaccine in the past ten years
  • We recommend with the Pertussis vaccine for adults with exposure to children under 18 months
• Measles Mumps and Rubella  (for all except born before 1956)
• Influenza Vaccine (all applicants October 1 to April 1)
• Hepatitis B Vaccine (or antibody test) up to age 65    (As of May 20, 2022)
• COVID - required per requirements of CDC guidelines
• Varicella Vaccine or history of Varicella - chickenpox ;  An antibody blood test can be done for confirmation.

* Senior Citizens over 65 years require a pneumonia vaccine-- Prevar 20

For Children under 18 years:
School records usually satisfy requirement

• Hepatitis B  -- three vaccines
• Polio
• H Influenza
• Meningitis (age 11 to 18)

* Please note many pediatricians give this to children at 12 years of age.
